What is Service Coordination?

Service Coordination is a way to link, organize and arrange services, resources, and supports to assure that families have access to people, places and resources that make a difference for them.

Service Coordination brings together all of those working with a youth & family to work together to assure that services are aligned, coordinated and streamlined. The steps in the process include:

How It Works

Children and youth between the ages of 0 and 21 who are already involved in 2 or more child serving systems can receive Service Coordination. Involvement in juvenile justice, children services or welfare does NOT prevent access.

All families are unique and have different strengths, talents, and experiences. Service coordinators help families recognize their potential and help uncover the unmet needs that are causing concerning behaviors.

Navigating multiple systems is often confusing and frustrating. Service coordinators help bring together all relevant service providers to work towards common goals. Families and providers are able to stay in the loop and work better together.

Service Coordination is not a one size fits all option. Each family will help develop an Individualized Family Service Plan - the roadmap to achieve their vision. As the most important members of the team, families are in the driver’s seat. As helpers, service coordinators are committed to providing the best information available and making adjustments as needed.
